COIDA Claim Forms: WCL 2, WCL 4, and WCL 5 Explained
Correct paperwork is the foundation of every successful COIDA claim. Three forms form the core of the claims process.
Form WCL 2: Employer’s Report of Accident
The employer must complete WCL 2 within 7 days of the accident. It records the date, time, place, nature of injury, and witness details.
Form WCL 4: Doctor’s First Report
The attending medical practitioner completes WCL 4 after the first examination. It describes the injury, treatment plan, and estimated incapacity period.
Form WCL 5: Final Medical Report
Once treatment concludes, the doctor submits WCL 5 stating the final outcome, permanent disability percentage, and fitness for duty.
